Inside a forensic psychiatric unit, cameras go where they have never been allowed before, into wards holding more than 100 patients, many committed for serious acts of violence and judged too mentally disordered for prison. The film follows staff managing daily risk alongside patients working through therapy sessions, medication routines, and the long process of reckoning with what they have done. Its central thread is discharge: how a panel decides, after years or decades of confinement, whether someone who once caused terrible harm can safely return to the outside world. Interviews with clinicians lay out the clinical and ethical calculus behind that decision, while patients speak candidly about their crimes, their diagnoses, and their hope for release.
