Madagascar's Ministry of Health declares an epidemic alert in September 2013 after the plague resurfaces in remote villages across the country, killing dozens within weeks. VICE traces the outbreak to its rumored origin in overcrowded prisons, where poor waste management and minimal medical care let the disease take hold before spreading outward. The film shows the country's rudimentary healthcare system straining against a disease that modern medicine already knows how to beat: antibiotics that treat the plague effectively exist and are common elsewhere, but Madagascar's infrastructure cannot get them to the people who need them. Poverty compounds the problem at every step, from sanitation to transport to the reach of public health workers into isolated communities.
