Boston has the highest rate of opiate overdose in the country, and Vanguard correspondent Mariana van Zeller goes into the city's neighborhoods to find out how it got there. She traces a pattern that shows up across New England over the past decade: painkillers like OxyContin, prescribed for chronic pain and widely available, becoming a gateway to heroin once regulation made pills harder and more expensive to get. Van Zeller talks to addicts and their families about the switch, a cheaper, more potent drug filling the gap left by a tightening prescription market. The film follows the practical mechanics of that shift, how supply and price on the street change user behavior, alongside the human cost, families strained and neighborhoods affected by the spread of addiction. It also looks at the healthcare system's role in over-prescribing painkillers in the first place, and the gap between that system and the treatment addicts need once dependency sets in. The focus stays local and specific to Boston and New England rather than treating the epidemic in the abstract.
