Since civil war broke out between the Sudanese Armed Forces and the Rapid Support Forces in April 2023, hundreds of thousands of civilians have fled to the Nuba Mountains, a remote region without paved roads, mobile coverage, or more than a handful of medical facilities. The film follows Joseph Yacoub, director of one of the region's few hospitals and its only surgeon, through days that bring hundreds of patients: women in late-stage labor, wounded fighters from both sides, and mothers carrying malnourished children. Not everyone survives the wait. Yacoub, who taught himself surgery on the job because the Nuba Mountains has no medical schools, is training Nidal Difan, who hopes to become the region's first female surgeon, and Aziz Burma, a former soldier turned medical trainee, so the hospital can keep functioning after him. Cameras stay close on the wards and operating room rather than the front lines, which have so far spared this particular corner of Sudan even as famine, displacement, and 25 million people facing acute hunger define the wider war.
